OBTAIN A REPEAT PRESCRIPTION?

Please complete the form below and submit to order your prescription online. You may also order your prescription by ticking the items you require on the form on the right-hand side of script. This can then be put in the box in reception, or posted to the surgery, enclosing a self-addressed, stamped envelope for its return. If you prefer, you may telephone your request (on 01527 599768) between 1.00 and 3.30pm daily. Please allow two working days for prescriptions to be generated. Remember that the prescription line is very busy on Monday afternoons.
